Last night I completed the survey which took about 20 minutes. It appears the company behind the scenes is a pharmaceutical or distributor for Bayer and other products.
In the beginning, the survey will ask if you received this offer at a doctor's office or if it was passed along by a friend. If you answer doctor's office, you must have visited him/her within the last 3 days. Then the survey will ask for the zip code and phone number of your doctor. Results are confidential. It will take 6-8 weeks to receive your $5.00 check. Only one per household.
